Daniel Agger is out for Liverpool, while Aston Villa have defensive injuries of their own.
Agger requires surgery on a back problem and will be missing for six weeks.
Also doubtful is centre back Martin Skrtel, who has a fractured jaw, so new signing Sotirios Krygiakos is favourite to partner Jamie Carragher in the centre of defence.
Insua will again start at left back, with Glenn Johnson providing width on the right. Fernando Torres will be upfront alone, with support coming from Steven Gerrard.
For Aston Villa boss Martin O'Neill, an injury to central defender Curtis Davies means new signing Habib Beye will deputise in central defence.
O'Neill is hopeful that Norway international striker John Carew will be fit enough to start, having missed the first two matches with a foot injury and a virus. Carew could replace Emile Heskey to partner Gabriel Agbonlahor in attack.
